Just a question, but if he were traded does anyone know if the 5th year extension goes with him to the new team?

These Garoppollo and Bradford comparisons are apples and oranges compared to Blake's situation. Garoppollo is a QB that's not only been allowed to sit and be properly groomed, but he's been allowed to do that in what should clearly be considered the best place in football to do that. He's also looked really good in limited snaps and was no slouch of a pick coming out of college.
Bradford was always supposed to make the next leap but his career has been riddled by injury. He's looked very game managerish when he has been healthy which is more than can be said about Blake right now and the situation last year in Minnesota was a perfect storm for his trade. The Vikings thought they had a super bowl caliber roster and game manager Teddy went down. Bradford didn't play poorly last year it's just that the decline of Peterson came a year too early for them. I don't know that I would have paid the price they did for Bradford, but I understand the move.
Bortles isn't solely responsible for the Jags record since he's been here, but he's clearly been largely responsible for losing more games for us than he has won. Nobody is paying a premium for that.

I was wondering the same thing but I vaguely remember reading that it doesn't, but don't quote me on that.

Bortles is not solely responsible for their record. Last year maybe, but the prior season was on the defense. Blake was constantly putting the Jaguars in position to win, but the defense just as constantly let them down. Blake showed tremendous skill that season racking up the TDs and stats, and making the Jaguars offense formidable. Granted he struggled last year, but he's still EASILY worth a second round pick.

For the most part you should at least get a 2nd round pick for a starting QB. Sure there are exceptions for an older guy or someone who really shouldn't be starting, but Blake is not one of those exceptions.

Still 5-11 and blame the backs, and defense. But remember the mojo rushing title and top 10 d and still not 500. Ya have a clutch qb that wins games or you don't. Simple as that.
