Based on how things are likely to play out, I think this would be the situation.

Ravens will beat Bengals next week giving them the #5 seed.
Jags will hold tiebreaker over Kansas City for #3 seed even if tied at 10-6 because of a better conference record.

If Jags beat the Titans in week 17, Titans are out of playoffs. Jags would play the Chargers if they beat Raiders. If Chargers lose, and Bills win, then Jaguars play the Bills.
If Jags lose to Titans in week 17, they will replay the Titans in Divisional Playoff game.
