If you get a bill at a restaurant for $50 and have a $50 gift card added to the bill, you would only tip on $50 right?

What would be the alternative?  Leaving extra gratuity since the meal was a "gift?"  I, mean, I guess if you're feeling caught up in the spirit of the holidays and such....

The services offer the disposition of "you don't need it" but the patient/service user can decline that disposition and therefore the patient gets what the patient wants.

And there's your overutilization leading to lack of services and higher costs. The best way to prevent that is out of pocket cost at the time of service. Price pain leads to better decisions.

I just noticed the urinal in this anti drinking and driving commercial for the NHTSA. I've never seen a urinal situated next to the door. How awkward.
