They'll have a sensor that can read alcohol on your breath. Or a sensor will be built into a phone or smartwatch and you use an app to start your car.
Those are too easy to fool though and the legality is established to stop the physical device if you haven't been previously convicted.

Using cameras, lane sensors, etc could alert police of your potential drunkenness and may find a way to make it legal to report it.
