It probably was mostly just Coughlin, but he did seem to call out Caldwell in the final meeting they had.  It was probably a 95/5 thing in favor of Coughlin with Caldwell being the final nail in the coffin.  At least we can continue to put to bed the notion that Marrone was in any way an issue.

Even in that story Ramsey called Dave the bad cop, and implied Dave didn't usually act that way.  
Jalen demanding a trade didn't hurt the team directly.  The team got valuable picks from that.
It was the indirect damage.  The confirmation that the team did not treat players well, causing free agents to not want to come to Duval or stay in Duval, and hopefully that's over now.  Time will tell.
