He must be challenged to make some tough decisions. If he is loyal to a fault, his fault might lead to his departure (again).

He is ultimately accountable. He was brought in based on his ability to lead, to manage, and to get results out of the team that at this point was built for him. If that is not happening, he needs to either change his approach, change his support staff, or step down if unwilling to make those changes.

I am not the biggest fan of volatility. Change for the sake of change or setting unattainable expectations is not going to foster success often. But the way things have started, there must be SOME change made to address the failure. We may not take drastic action right away, but he's got to get a hold of things and start recuperating the team's confidence or we will spiral quickly.
