There is a simple and obvious possibility.  He may give each new patient a questionnaire in the introductory session, telling them to answer as much or as little as they wish.  The information could be used to help guide his therapy with each patient, or for statistical research only, without any patient identification.

At some point they'll probably figure out political leanings based on random info the patient gives. Hell, you can just look at some people and tell which side of the aisle they sit on. Back in the 90s I had a shrink who was full on hippie chick. Definitely liberal but not the hateful kind we see now. 

If a shrink (or any doctor) ever does start publicly talking about "conservatives are crazy!" or "liberals are crazy!" they won't be a practicing shrink for very long.

But that's not what Sneakers said. 
Sneakers said that they might directly ask political questions on a written questionnaire, but with an "answer as much it as little as you want" comment. They most definitely do not.

LOL.  The word political does not even appear in my comment, nor did I say anything about specific questions.

"I'm collecting information for statistical research. There are blank questionnaires on the table, feel free to fill one out if you wish.  Answer whatever you're comfortable with and don't put your name on it, all information is anonymous   Just drop it in the box after you're done."

It's really that simple, you just can't admit there's even a chance that you're wrong.
