Are you now agreeing that the therapist may have conducted such a survey, and are only disputing the accuracy of his findings?

If he had done a comment box style survey, the results would be inaccurate, and he would know that they were inaccurate. But he represented his results as being accurate. So this scenario also falls under the "he lied" possibility. 
As I said, there are really only two possibilities, either he was unethical, or he is lying.
