To me its on Telvin both times. He is not expecting the underneath coverage to hold up and he is giving up his depth on his zone, and reacting to the underneath coverage(not covering his zone and letting a guy get behind him). This is a prime example of trying to do too much/not trusting your teammates to do their job.
