If I had to rank 'em using the OP's criteria:

 

1. K-38/Circa 1922 from my current hometown.  Not exactly upscale, but a step above sit-down chain. And delicious.

2. Bonefish Grill

3. Publix subs

 

As far as my most unforgettable dining experiences?  Maria Restaurant in Brooklyn, Mamaronack Oyster House (Mamaronack, Long Island), "Boston Chicken" in Port Antonio, Jamaica, Boujou's Pizza in Idaho Falls, Colorado, and some hole in the wall in downtown Puerto Vallarta, Mexico.

If a Mom and Pop grill with potential wait times close to an hour on certain nights (local high school homecoming) can be classified as fast food, I'd like to amend number three to Walker's Video and Grill. Best burgers and pizza on the mountain, plus you can rent a DVD for the night. Who still does that?

For high-end I'll have to go with Dwight's Bistro on Penman at the Beach,

For sit down, Sliders on First Street, at the Beach,

For Fast Food, Home Plate Diner, on Mayport Road, (You guessed it) at the Beach.

The owner is the original Colonel Mustard.

His burgers and onion rings are great!

I had Ted's once. And it was the biggest waste of money in my life. And I've been to some crappy places. My bill was like 70 bucks for me, my ex, and my son on food only and it was terrible.

Fine- I don't know if I have a "fine dining" choice. I have rarely fine dined, especially on my own nickel. Maybe not exactly "fine dining" but I would choose any of the revolving restaurants I've had dinner in- Cincinnati (actually in Covington, KY), St. Louis, Las Vegas (Stratosphere- wow), Phoenix, Cambridge (MA, now closed)- can't beat the view from up there.

 

Edit: I'll go with Emeril's in New Orleans, been there a couple times, best dinners ever.

 

Casual- A1A Ale House in St, Augustine

 

Fast Food- any number of good pizza places in Boston area; also I agree with Publix subs as someone mentioned above

Lol maybe but long horn is what we call fine dinning. Hell we went to a "fine dining" place once I think it was called Avendales it's on riverside. Took the wife there when we got engaged it was 50 bucks a plate and the food was so little! It was like a kids portion of steak and potatoes we hit Wendy's up on the way home.
