Whose fault was the miss? Regarding the first part of what you mentioned, Blair Walsh appears to be an exception to the rule and is now unemployed as a result.

Now, he can't be counted on routine kicks and he even didn't execute a kickoff the way he was told 9 days ago. Whether Walsh can turn things around if given another opportunity remains to be seen.
