Has any player failed to get reinstated? Is Blackmon continually tested? If so, what if he continually fails the tests?

What if he doesn't apply for reinstatement because he can't get clean? Does he just fade away? Or can he eventually be released without qualifying for reinstatement?

Tanard Jackson was suspended for two years and just got re-instated. Johnny Jolly was suspended for three years and was allowed to return last year.

I think the timetable is up to him in terms of taking the necessary steps. Based on what Bradley said Friday night, it does not sound promising.
