At least one of the 3 phases (offense, defense, special teams) fails big time in every game.  There is no one else to hold accountable for that than the head coach. This isn't a new phenomenon. Coaches who fail to get the entire team to execute get fired all the time.  I like Gus and the culture he brings but he just doesn't what it takes to be a head coach in the NFL.  He would be best to go back to being an assistant or a head coach in college where the game isn't nearly as complex.
