Medical schools can sprout up but what is restraining the output is number of residency seats available here in the US, which is regulated by the ACGME. This issue of #medical students to #residency slots available is already occurring. 

 

DO's can enter a DO residency, as well as an MD residency (but is generally harder for the higher-tier specialties).

There will be no more DO residency in 2020, they will all be one after the merger. And while new residency's are being added, they are pretty much exclusively primary care. ACGME reminds me of nursing schools a decade or two ago- Shortage shortage everywhere! Then they doubled the amounts of nurses being produced and when the market crashed suddenly new grads couldn't get jobs in a 'recession proof' profession. I was there, I remember it well.

 

The residency is the restriction, but we have a problem with geographic distribution of providers more than anything else. People want to live and work in the big population centers with the money and support. Also they want all the normal things that people want (good schools for kids, sports teams, costco etc). Most people do not want to work at some dinky hospital with less than 100 beds where the patients cry when they found out they were dropped there. Where you are basically a referral center for your flagship.

 

Why should someone proposing to build a hospital have to prove to some government bureaucrat that it's "needed?" Can't a better hospital with lower fees be good for everyone, whether or not there is already a hospital in place?

I can see the other side of the coin, hospitals only want to add a footprint where the moneys at. So instead of going to a medically needy area that's further out and poorer, they all want to get in the same middle upper Surburbia. When you have a resource like medicine you can't just let them line up next to each other like car dealers. People need emergency care in different places.

 

But overall I agree, you should be able to put were you want.

Either it has lower fees or better health care. If the new hospital was no better than the old one no one would go there and it would fail. But that risk is on the builder, not the citizenry. Competition breeds lower prices and/or a better product. Government-enforced monopolies breed high prices and stagnation
 (all monopolies do, but here we're talking about government meddling).

If there were no money in healthcare than you would have a big brain loss. You could cut a lot of managment out and not feel it, but if you lose the Docs and the research, we would feel that. There are not alot of professions that I think are more important than healthcare. Profit is not bad, and isn't evil in itself, but we don't have a real capitalistic based healthcare system. Its more like an oligarchy. And that tends to lead to price gouging. Especially when the government gets involved as a payer. Nothing the government touches goes down in price.

This is what happens when you don't have Fee for service or universal healthcare. The middle tends to combine the worst aspects of both. If it was all cash things would be cheaper, and you would have more control over your care. If it was universal you would have much less control but wouldn't go bankrupt. As is you don't have good control for the most part, and you still get to go bankrupt.

You can't take the profit out of healthcare.   Research and development are very expensive, and if the companies doing the R&D cannot sell the results for a profit, they won't do it.  

 

I don't believe there will ever be a "solution" that makes health care less expensive, for the simple fact that people will pay anything, and I mean anything, to save their own life.  

 

About all we can do is decide if we want to help poor people afford health care, and are we going to spread the cost among the general population of the country.  That's what the arguments about Obamacare, repeal and replace, etc, are really all about.   But the politicians all try to sell their ideas on the basis that they are going to make health care less expensive.  That is absolutely impossible.
