The residency is the restriction, but we have a problem with geographic distribution of providers more than anything else. People want to live and work in the big population centers with the money and support. Also they want all the normal things that people want (good schools for kids, sports teams, costco etc). Most people do not want to work at some dinky hospital with less than 100 beds where the patients cry when they found out they were dropped there. When you have a resource like medicine you can't just let them line up next to each other like car dealers. People need emergency care in different places. Wanted to add a quick note thanking everyone for their feedback. I will be adding my opinions in the next day or so, both general and with a bit of feedback from my recent efforts in setting up my primary care provider (switched insurance with my employer in December). I guess the short version is based on my recent experience my dog could build a better health care system (and I don't even have a dog). Good people in the system, but the mechanics suck.

If there were no money in healthcare than you would have a big brain loss. You could cut a lot of managment out and not feel it, but if you lose the Docs and the research, we would feel that. There are not alot of professions that I think are more important than healthcare. Profit is not bad, and isn't evil in itself, but we don't have a real capitalistic based healthcare system. Its more like an oligarchy. And that tends to lead to price gouging. Especially when the government gets involved as a payer. Nothing the government touches goes down in price.

If it was all cash things would be cheaper, and you would have more control over your care. If it was universal you would have much less control but wouldn't go bankrupt. Research and development are very expensive, and if the companies doing the R&D cannot sell the results for a profit, they won't do it. I don't believe there will ever be a "solution" that makes health care less expensive, for the simple fact that people will pay anything, and I mean anything, to save their own life. About all we can do is decide if we want to help poor people afford health care, and are we going to spread the cost among the general population of the country. That's what the arguments about Obamacare, repeal and replace, etc, are really all about.
