The first one actually happened. I’ve seen video of it. It occurred on a construction site in China. I don’t recall if the victim died.

I've seen aftermath photos of the second one.  Poor guy was standing over a spinning shaft with his safety lanyard hanging.  It got caught and pulled him in, the harness did indescribable damage to his torso as it wrapped and tightened around the shaft.  It was horrid. 

Several of the others I've seen as well.  I worked in industry and they would show these events in the safety meetings.  It really drives home the , it could happen to you message seeing someone go through it and perish.
